About Kelly Hilaire
Kelly Hilaire is a board-certified Family Nurse Practitioner (FNP) fully licensed to practice in the state of New York, providing Home-Based Medical Care, also known as House Call Services, to patients in New York City. He is qualified to treat a wide array of diseases and conditions across an individual's lifespan. He graduated at the top of his class with honors from his baccalaureate studies through his graduate studies as an FNP. His educational journey began at the Brooklyn Campus of Long Island University (LIU), where he earned his Bachelor of Science in Nursing. After working more than a decade as a Registered Nurse, Hilaire returned to LIU to complete his Master's in Nursing.
Hilaire's career began in November 2010 when he accepted a position at Columbia Presbyterian on a surgical unit. He then went on to work for the Health and Hospital Corporation (HHC) in a variety of inpatient settings, including Forensic Psychiatry, Psych-Emergency Room, Medical-Surgical-Telemetry, and Step-Down ICU. One of the biggest dilemmas that has intrigued Mr. Hilaire in his practice is the revolving door phenomenon. He has always been perplexed and frustrated by the constant readmission of patients to the hospital for conditions he believed could easily be managed at home.
After his graduate studies, Hilaire decided to meet patients where he believes it matters most-within their homes-hoping to make a minor dent in the constant inpatient readmission crisis. Thus, as an FNP, he has made it his aim to work as a Home-Based Medical Care provider through various employers, including Essen House Call, Prospero Health, Landmark Health, and Optum at Home, doing exactly what he believes is part of the solution. Now, he has decided to begin his own practice in collaboration with his physician colleagues and other NPs, PAs, and nurses, hoping to help patients live longer at home, reducing visits to urgent care, emergency rooms, and inpatient settings.

I just wanted to share how much I truly appreciate the care I’ve received at Vine NP Health. Kelly Hilare is an amazing provider. He really takes his time during appointments and never makes me feel rushed. I always leave feeling like my concerns were genuinely heard and addressed.
He explains everything in a way that’s clear and easy to understand, and he makes sure I’m comfortable and informed about my care. He’s such a great listener, and that means a lot. With him, I don’t feel like just another number I feel seen, respected, and cared for.

Doctors Kelly and Marlie are wonderful practitioners, who always inform me on the best options that are available to me in regards to improving my health. This provider took care of my brother as if it was his own family member, prior to utilizing his serviced we have never used House call services and through this outstanding experience, i am now convinced that this is a greater alternative for my family.
