About Dr. Johnique Bennett

Dr. Johnique Bennett is a board-certified Family Medicine and Obesity Medicine physician and the solo clinician of Affirm Direct Primary Care, a comprehensive medical clinic serving patients ages 12 years and older for all primary care needs. She provides medical care and coordination for members through a subscription starting at $75 per month.
Dr. Bennett earned her Doctor of Medicine and holds a Master's in Health Science from the University of South Florida, and completed her undergraduate studies at Florida State University.
Affirm DPC uses a subscription-based payment model to provide access to healthcare at an affordable cost for insured, underinsured, and uninsured populations alike. For a flat monthly fee, we DO NOT accept health insurance as a form of payment.
Dr. Bennett currently offers three service packages: Monthly memberships for adolescents 12-17 years ($75 per month) and adults 18 years and older ($99 per month), and a one-time general check-up for $265, which includes a head-to-toe exam, basic labs, and a screening EKG.

I had my first appointment with Dr. Johnique Bennett today, and I couldn’t be more impressed. As someone who has never had a general practitioner, I wasn’t sure what to expect, but from the moment I walked in, I felt welcomed. After a thorough vitals check, Dr. Bennett introduced herself and took the time to explain the basics, diving deeper when I needed clarification. She was incredibly thorough, genuinely invested in my overall well-being, and had a remarkable ability to break down complex medical terms in a way that was both informative and easy to understand. Most importantly, she was never pushy or judgmental—she met me where I was and made me feel completely at ease. I couldn’t recommend her enough and am actually looking forward to my next visit!

I was thoroughly surprised with the level of attention I was given. It felt so natural talking to Dr Bennett & I enjoyed every part of my experience. She listened and responded, I simply felt heard. Also, before prescriptions are suggested she definitely offer the more natural approach first which I appreciated even more.

Dr. Bennett was great. She was attentive to my needs and provided great advise. She was very accommodating and made our appointment stress free all without having to wait. I ran into an issue with the pharmacy and she was available to help me immediately resolve it and provide alternatives. Being able to communicate with my doctor as I have with Dr. Bennett is rare. I highly recommend her to any patient looking for a Doctor who cares and is there to help and listen. She spends the time to understand what is going on in your life in order to better diagnose your medical situation. Her subscription service is great and will save me money and gives me the piece of mind knowing she is available to answer any questions I have without having to deal with receptionists and long wait times. Thank you, Dr. Bennett!
