Who is a family physician?

Family physicians are trained to provide continuous diagnosis and treatment to every family member, irrespective of age, gender, or condition. Generally, families call upon such specialists in case someone falls sick. Families typically have a dedicated family physician, which underlines the importance of finding the right practitioner.

They can provide continuous support for chronic conditions, study symptoms, prescribe tests, conduct routine checkups, prescribe positive lifestyle choices, and more. To become a family physician, aspirants start by completing an undergraduate degree. Then, they get one of the degrees—a Doctor of Osteopathic Medicine (DO) or a Doctor of Medicine (MD) — from an accredited medical school. After completing a doctorate, aspirants join a residency program to get the experience needed to start their practice.

Regular consultations with a family physician allow you to catch severe chronic conditions early. In addition, if you're diagnosed with a severe disorder, you can get references from specialists.

Subspecialties within family medicine

Family physicians may undergo further training to specialize in any of the following family medicine subspecialties

  • Hospice and palliative medicine: The subspecialty deals with patients suffering from life-limiting illnesses. These specialists work with the palliative or hospice care teams to address the patients' and their families' social, physical, spiritual, and psychological needs and improve the patient's quality of life.
  • Pain medicine: This specialty treats patients suffering from chronic, acute, or cancer-related pain in outpatient and hospital settings. Its specialists also coordinate with other specialists to optimize patient care.
  • Adolescent medicine: It is a multidisciplinary healthcare specialization that focuses on the unique social, physical, and psychological characteristics of adolescents, their ailments, and their healthcare needs.
  • Geriatric medicine: This specialty deals with elderly care. It focuses on specific skills to prevent, diagnose, manage, rehabilitate, and treat conditions in older adults. These specialists can care for elderly patients at home, offices, hospitals, and nursing homes.
  • Sleep medicine: This subspecialty focuses on diagnosing and managing sleep-related clinical conditions that affect sleep quality or disturb the normal wake-sleep cycle. Specialists can interpret and analyze comprehensive polysomnography and effectively manage a sleep laboratory.
  • Sports medicine: This focuses on preventing, diagnosing, and treating sports and exercise-related injuries or conditions. It also deals with diseases and illnesses that affect physical performance.
  • Healthcare Administration: The subspecialty equips a physician with comprehensive expertise in managing administrative functions in healthcare. They focus on organizational effectiveness, patient experience, process improvement, and patient safety. These specialists work with stakeholders to create a robust, patient-centric healthcare system.

What sets internal medicine apart from family medicine?

The distinction between internal and family medicine can be puzzling for medical students and patients. Although they may appear similar, these two disciplines exhibit notable differences. Internal medicine emerged in the late 1800s, emphasizing scientific application in medical practices. Concurrently, pediatrics, a field centered on children's healthcare, flourished in the early 1900s, leading to internal medicine's greater focus on adult patients.

The late 1960s witnessed the inception of the general practitioner movement aimed at preserving solid doctor-patient relationships and promoting continuous care. This movement gave rise to family physicians, specialists well-versed in addressing diverse medical conditions affecting any family member.

While internists primarily cater to adult patients (though some may also handle pediatrics), family physicians must have expertise in adults and children. Internal medicine practitioners devote equal time to training in inpatient and outpatient settings. In contrast, family medicine places a primary emphasis on outpatient care. In addition to their specialized training, family physicians address behavioral health issues, skin disorders, preventive medicine, overall well-being, population health, and health system management.

Does your family need a family physician?

If you live alone, booking an appointment with a family physician near you for your symptoms is feasible. However, taking the same approach to managing your family's health is not viable. Every person in a family requires a different level of medical attention. Hence, here are some reasons why you should find a family physician.

  • Lifecycle management: A family physician who treats a child from birth through adolescence and adulthood would have an accurate idea about the person's medical record. Such a practitioner would know specific medical details like allergies, past conditions, chronic diseases, and other information associated with a person's medical history. Thus, family physicians can serve as your family's storied medical advisors.
  • Enormous scope: Family physicians don't simply treat seasonal flu and coughs. They can also manage immunizations and severe conditions like arthritis, chronic pain, and heart disease.
  • Recommend the right specialist: If you have been diagnosed with a severe disease, you may need help finding the right specialist. Family physicians can suggest the best specialist for your needs, as they have an in-depth understanding of your medical history.

What do family physicians do?

Given family medicine's enormous scope, it can take time to identify the primary diagnoses and treatments that family physicians offer. Here are a few key responsibilities and procedures that family physicians can manage:

  • Chronic disease treatments: Family physicians can treat and monitor chronic diseases like high blood pressure, diabetes, and arthritis. They can prescribe or update medication based on how patients respond to treatment.
  • Injury recovery: In case of a physical injury, family physicians can help monitor your recovery process and ensure proper healing. They can prescribe pain-relief medication and check on the healing of your wound.
  • Routine checkups: Family physicians offer regular health checkups to monitor your health in the long run. These checkups are crucial for family physicians to catch chronic diseases early and adequately start treatment.
  • Lifestyle advice: Adopting positive lifestyle choices like losing weight or quitting smoking sounds easy. However, with sound medical counsel, family physicians can help you transition to a healthier lifestyle.

How to prepare for your first family doctor appointment?

Preparing for your first family physician appointment will help you get the most out of your visit, including receiving preventive care on time and saving on future medical costs, ultimately leading to better health outcomes.

Here's how you can go about it:

  • Ensure you bring all relevant documents: This includes all your previous health records, prescriptions, insurance card, a list of over-the-counter medications you are currently taking (including vitamins and supplements), and prior tests or surgeries performed. If you have ever been treated in an emergency department by a specialist, inform your family doctor. These details will help your provider diagnose accurately and keep your overall health in mind for future visits. Further, you must also notify your doctor if anyone in your close family has a history of chronic illness.
  • Maintain a record of all your symptoms: It is essential to note all your recurring symptoms, their frequency and duration, and what triggers them. Changes in your diet, sleep patterns, weight, moods, and menstrual cycle could indicate an underlying health condition.
  • Ask someone to accompany you: If you are bilingual or English is not your first language and need help translating, you can ask a friend or family member to translate during your appointment. This can also help you calm your nerves and feel less stressed during the session. They can also take notes on your behalf and raise any questions you may have forgotten.
  • Prioritize your concerns and carry a list of questions: Your first appointment may take quite some time. You may also have many questions regarding your diagnosis, treatment options, tests needed, surgical options, medical costs, follow-up appointments, and more. It is advisable to carry a list of these to ensure you remember all critical queries while you are with your doctor.

What to expect when you first visit a family physician?

When you visit a family physician near you, they will inquire about your health-related concerns and lifestyle. They will start by posing questions to gather information about your health. Additionally, they will ask about your symptoms, current medication usage, and your family's medical history. During the appointment, your doctor might take the following actions:

  • Assess your vital signs, encompassing blood pressure, body temperature, heart rate, and breathing rhythm.
  • Ask for regular blood tests to screen for conditions like diabetes.
  • Examine the head and neck.
  • Examine the abdomen.
  • Administer immunizations.
  • Conduct a skin examination.
  • Conduct health screenings for women, including pap smears, osteoporosis assessments, mammogram referrals, and sexual health evaluations.
  • Carry out health screenings for men, including prostate exams, aortic aneurysm checks, and referrals for colorectal exams.
  • Screen infants and children to evaluate their growth.

If you have a specific condition, they will inquire about your symptoms and current management strategy and recommend lifestyle adjustments to alleviate them.

It's essential to ensure that your family doctor understands your health condition and perspective. If you have any new troubling symptoms or queries about vaccines like flu shots, it's advisable to list these concerns. Additionally, including your health objectives on the list will enable your doctor to propose lifestyle changes to help you achieve them, such as adopting a healthier diet or engaging in joint pain-free walks with your loved ones.

Having a loved one or family member accompany you for your initial appointment can be advantageous. They can assist in note-taking or remind you to ask specific questions during the visit.
